The Standard Waiver is required for all recreational education and dive events. 
 

 

The Tech Waiver is required for technical training and dive events. 
 

 

The Medical Evaluation Form is required for all training, and may be required for dive events as well. If there are any questions regarding

the medical fitness for diving of any potential participant, a doctor's signature may be required, at the discretion of the instructor or

dive master.
